California Consumer Privacy Act Disclosure

The California Consumer Privacy Act (“CCPA”) creates privacy rights relating to the collection, sale, disclosure, and deletion of consumers’ personal information. The CCPA requires businesses to provide consumers, including job applicants and employees, with information about their rights, including a description of the categories of personal information to be collected and the purpose for which the information will be used. This Disclosure Form is intended to advise you of this information.

What Information Is Collected?

Catalight and its Family of Companies, including Easterseals Northern California (“the Company”) will collect the following categories of personal information from you:

Characteristics of Protected Classifications under California Law For applicants and employeesThis includes data such as gender, ethnicity, disability, and veteran status collected through required Equal Employment Opportunity and Affirmative Action forms.
New Hire/Onboarding Paperwork For employeesThis includes your resume, job application, IRS Form W-4 and W-2, USCIS Form I-9, CA State Form DE 4, banking information, auto insurance verification.
Pre-Employment Screening Paperwork & Results For applicants and employeesFor certain job positions, this may include health screening results and motor vehicle records.
Pre-Employment Background and Credit Checks and Results For applicants and employeesThis includes post-offer, pre-employment background checks for all job positions. Credit checks will only be performed for certain job positions in the Finance Department in accordance with state and federal law. The information collected may include identifying information, certain criminal records, information relating to your employment and education background, and for credit checks: your credit score, credit accounts, and credit inquiries.
Timecard & Payroll Data For employeesThis includes time worked and identifying information for purposes of determining compensation.
Leave-Related Documentation For employeesThis includes paperwork related to protected leaves of absence under the FMLA/CFRA, or use of sick leave, vacation time, or wellness days.
Employee Benefit Plan Information For employeesThis includes identifying information maintained for the administration of the Company’s health, dental, vision, retirement, life/AD∓D and disability plans.
Employee Activity For employeesThis includes your activity on all Company networks and communication systems and your activity on all Company-provided devices, including smartphones and laptops. Your “activity” includes your browsing and search histories, your communications with others, text logs, geolocation data associated with your use of a Company-provided device, and all information transmitted by, received from, or stored in a Company system or Company-provided device.
